InfoReady accommodates digital signatures at the end of both application forms and review forms. The term used in the system for digital signatures is "Acknowledgement Statement". This is a great option to use if you ever need acceptance for conflict of interest statement, acknowledgement of requirements, or any other similar affirmation.

When an Acknowledgement Statement has been activated, the end user will see the statement provided at the end of the form and must check the box next to the statement to submit the form. If the user does not check the box, then the Submit button will remain deactivated and appear grayed out.


Where to activate Acknowledgement Statements:


Application Form: Found on the Requirements page when creating or editing:

Applicant acknowledgment section. Red arrow pointing to text: "I do not want to include an acknowledgment statement" with explanatory red text: "Check this box if you don't need an applicant acknowledgment statement." In text box below, red explanatory text states: "You can customize text here, such as conflict of interest statement, honor code, etc."

Review Form: Found on the Routing Steps page when creating or editing. This can be activated per routing step, meaning that a review process with multiple steps could have different acknowledgment statements for each routing step:

Reviewer acknowledgment section.
