Overview - Applicant can choose none, one, some, or all options from a list. - Can be used as an acknowledgement statement - 255 character max (for label and any response options)
Default Admin View
- Click directly in the label, instructions, and items fields to edit the text.
- Add choices (including Select All and None) with green plus signs.
- Remove choices with red minus signs.
- Click Settings > Choices to copy and paste a list in bulk (see "H" in next section) and configure other options.
Note: Be careful to put unique choices in the list of "Item 1, Item 2", etc. You are not able to rename "Select All" or "None" and should be careful when re-naming "Other".
Settings Panel Sections
General
A. Field reference - Unique identifier in the InfoReady database that does not show to applicants or reviewers.
B. Field label - Question label
C. Instructions (optional) - Any additional notes to help applicants answer the question
D. Required - Make the question required or optional (same as clicking asterisk * on main question screen).
E. Hide from reviewers (optional) - Check to hide the question and applicants' responses from reviewers.
F. Hide from applicants (optional) - Check to hide the question from applicants.
Choices
G. Eraser - Clear all choices
H. Edit - Copy and paste a list from another source (e.g., Word, Excel) to populate choices
I. Add - Add a choice to the list
J. Rearrange - Drag and drop to re-order choices
K. Visibility - Control visibility of a single choice (compare with overall logic in next section)
L. Delete - Remove the choice from the list
M. Choices sort order - Choose None (leave as shown), Ascending (A-Z), Descending, (Z-A), Random
N Allow the Other option - If checked, text can be customized
O. Allow the None option - Lets applicants indicate no options are relevant with one click
P. Maximum selected choices
- Default is 0, which actually means they can select as many as they need.
- You can also restrict applicants to selecting a certain number of options.
Logic
- Choose when the question is shown and/or required.

Layout
Q. Show on page - Choose which page of the form the question shows (Can also drag and drop from main screen).
R. Display field on a new line - Uncheck to show the question directly to the right of the previous one.
S. Field label location - Choose if and where the label should show.
T. Instructions location - Choose either under the field or under the field label
U. Column count - Choose how many columns to display choices in. Use more columns for longer lists.
Example Customized Applicant View
- From the Preview tab, here is how the question built above would look to applicants.
- Click directly in the label or instructions fields to edit that text.
- Switch back to the Designer tab to make any edits to logic or layout.
Example Acknowledgment Statement
- Use a one checkbox "question" with specific language in the instructions.
- For a more detailed statement, use a paragraph field (rich text) directly before the checkbox field.
