If you'd like reviewers to fill out a form as their review feedback, you can use the Document Upload routing step type. Administrators upload a blank or template review form that each reviewer then downloads, fills out, and submits as their review.
Administrator View of Routing Step Set Up
1. Under Routing Step Type, select Document Upload.
2. Upload the template review form under the Add Attachment section.
3. Confirm the correct file was uploaded. Then edit the Label, Instructions, and/or Name of the file (if needed).
Note: The Label field cannot be more than 45 characters (with spaces).
Reviewer View
Download link alerts reviewer to the file, which they then upload using the Choose File button.
Administrator View of Completed Review
Template file is above the reviewer box, and submitted forms are located in the Download Attachment column.
