Build My Own Form is available for Silver licenses and above. You can create much more flexible review forms with multiple question types.


An updated version of this routing step type for the Form Designer Template is currently being tested with a small group of administrators in early 2024. More information here: Form Designer Template: Build my own Form Routing Step (Beta)



Overview

  • Question labels and any multiple choice or multiple select responses must not exceed 255 characters (including spaces). If you need to add additional text, add the question first and then fill in more text in Note to Reviewer field.
  • If you build this routing step during the initial creation process, once any application is submitted, the individual question options cannot be edited.
  • If you create the routing step after applications have been submitted, make sure each question on the form is finalized before assigning any reviewers.

Here's how to use it:


 From the Routing Step Type dropdown, select Build my own Form:

Scroll down past the Reviewer Instructions to the form builder. The fields available are the same found on the Requirements page:

Question Types

Date – Applicants can choose a date from a calendar.

Multiple Choice Questions – Applicants select a single answer from a list.

Number Field – Applicants can only enter numbers.

Text Box – Applicants can enter long form replies and edit format. Admins can add character or word limits that are enforced by the system.

Text Field – Plain text entry with a 255 character limit

Yes/No – Applicants select either yes or no.

Multiple Select Questions – Applicants select all answers that apply, and admin can set min and max amount of choices.


As you add fields, they will appear in the box above:

You can use Build My Own Form to create review questions with optional text boxes, or no text boxes. If you want a rating only, with no comments, you can use a Multiple Choice Question for the rating. If you want a form with rating scales for each criteria and one text box for overall comments, then you can do that. too. Lots of flexibility!
